Linkworkers
What is a Linkworker?
 |
Linkworkers come in pairs |
 |
There is 1 to work between the
child and family |
 |
And 1 to work between the child
and school |
Linkworkers are experienced in working
with children and are able to work effectively with others
including parents and schools to provide planned and structured
support so that unwanted behaviour is replaced by wanted behaviour.
Who do they work with?

Linkworkers work with pupils
aged 5-11 years and their families and schools to prevent
and reduce exclusion from mainstream school with early
intervention |
 |
They help to develop positive
home/school relationships between parents and schools
caring for pupils with significant emotional and behavioural
difficulties |
 |
Linkworkers encourage and promote
the involvement of other services as and when necessary. |
Which schools are they in?
Linkworkers work in many schools within Heart of Portsmouth and Paulsgrove and Wymering.
What do they do?
Linkworkers are introduced to the
child, family and school where they then initiate the 10-week
intervention programme:

The programme starts with an
Initial Meeting. |
| 2. |
The meeting is followed by a
2 Week Assessment both in the home and in school. This
means that we listen carefully to what everyone is saying
and look at what they are doing so that we can decide
how to help. |
| 3. |
Then a Joint Action Plan (JAP)
meeting takes place. This meeting is to develop a joint
action plan of work which all parties involved agree on
targets and strategies that are a way forward to support
the child’s successes and that are realistic and
reasonable. |

In total around 35-40 working
days (7-8 weeks) of support is given by the Linkworkers
to ensure a continuous assessment of need and intervention. |
| 5. |
There is a Review Meeting after
week 10 to review the progress made since the Joint Action
Plan meeting and to plan ways of building upon the progress
made. In this meeting a decision will be made as to whether
further support is needed from other agencies. |
| 6. |
Exiting visits are made after
the 10-week intervention and 6 weeks after. An evaluation
form is filled in by the child, parents and school in
order to assess the support given. |
How do I access them?
To access the Back to School Support
Team a Request for Support Form is completed by the school
and signed by a parent or carer. The form is then sent to
the Primary Behaviour Support Service.
